Take this private invitation form to your local notary. Request they draw up a form using ONLY the text from this invitation form. Be sure that the notary stamps and signs on the same front page, NOT the reverse side or a separate page. We’ll just need a clear copy of this notarized letter.

Write the name of the person you’re inviting in Cyrillic and English like this: Джон Майкл Джонсон (John Michael Johnson)

The period of time the visa will be valid is up to 3 years. Put 3 years minus one day such as June 15, 2021 – June 14, 2024.

If the passport expires sooner than 6 months from the proposed visa end date, the private visa will not be for the full 3 years. You’ll need to put the visa end date as the passport expiration date minus 6 months and one day. For example, if the passport expires September 20, 2023, the visa will be granted maximum until March 19, 2023.
